Privacy Authorization, Medical Information & Tour Participation Agreement
Issued by Galápagos Redefined Highend S.A.S.

Dear Valued Guest,

At Galápagos Redefined Highend S.A.S., excellence, safety, privacy and world-class service remain our highest priorities. In order to properly organize, confirm, reserve and execute your travel arrangements and luxury expedition services within the Galápagos Islands and Ecuador, we respectfully request your written authorization and acceptance of the following terms and conditions.

By accepting this document, you acknowledge, authorize and agree to the following:

1. Authorization for use of personal information

You hereby authorize Galápagos Redefined Highend S.A.S., its representatives, affiliates, operators, suppliers and authorized travel partners to collect, process, use and store the personal information voluntarily provided by you for the sole purpose of executing and managing your travel arrangements and tourism services.

Such information may include, but is not limited to:

• Full legal name
• Passport information
• Nationality
• Date of birth
• Contact information
• Flight information
• Emergency contact details
• Credit card or payment information
• Medical and dietary information voluntarily disclosed by the guest

This authorization allows our company to process and use your information for purposes including, but not limited to:

• Hotel reservations
• Yacht and cruise reservations
• Expedition bookings
• Transportation arrangements
• National Park registrations
• Internal flight reservations
• Payment processing and authorized credit card charges
• Concierge services
• Luxury tourism operations and logistics
• Any additional travel-related services requested by the guest

All information provided shall be treated with the highest level of professional discretion and operational confidentiality.

2. Payment authorization

By accepting this agreement, you expressly authorize Galápagos Redefined Highend S.A.S. to process agreed-upon payments and charges corresponding to your confirmed travel services, reservations, tours, accommodations, cruises, transportation, permits or any additional services requested by you.

You acknowledge that all payments processed are directly related to the travel services voluntarily purchased and approved by you.

3. Medical information and health declaration

For your safety, well-being and proper assistance during your stay and expedition, it is mandatory that you provide accurate and truthful medical information prior to the commencement of your tour. Please disclose the following:

Medical conditions

• Existing medical conditions
• Physical limitations or mobility restrictions
• Heart conditions, respiratory conditions or chronic illnesses

Allergies, including but not limited to:

• Food allergies
• Medication allergies
• Insect allergies
• Environmental allergies

Medications

• Current medications being taken
• Important medical treatments
• Emergency medications required during your trip

Failure to disclose relevant medical information may compromise your personal safety and may limit our ability to provide adequate assistance during your expedition.

4. Swimming ability and personal responsibility

Certain activities within the Galápagos Islands may involve water-based excursions, snorkeling activities, wet landings, swimming opportunities and natural terrain conditions. For this reason, guests are required to inform our company whether they:

• Know how to swim
• Do not know how to swim
• Require flotation assistance or special supervision

The guest fully understands and acknowledges that it is their sole responsibility to provide truthful information regarding their swimming abilities and physical condition.

5. Compliance with Galápagos National Park regulations

All guests participating in tours, cruises, expeditions or activities organized through Galápagos Redefined Highend S.A.S. agree to fully respect and comply with:

• Regulations established by the Galápagos National Park Directorate
• Instructions provided by certified naturalist guides
• Safety procedures established by vessels, hotels, operators and expedition leaders
• Environmental conservation protocols
• Wildlife interaction regulations

Our company is deeply committed to protecting the extraordinary ecological integrity of the Galápagos Islands while ensuring a harmonious, respectful and unforgettable experience between our guests and one of the most extraordinary natural sanctuaries on Earth.

6. Guest acknowledgment

By accepting this agreement, the guest confirms that:

• All information provided is truthful and accurate
• The guest voluntarily authorizes the use of their information for operational and reservation purposes
• The guest understands the nature of expedition travel in the Galápagos Islands
• The guest agrees to comply with all safety instructions and environmental regulations
• The guest accepts personal responsibility for undisclosed medical conditions or limitations
